A care home at the centre of an alleged abuse scandal failed to meet eight out of nine standards set by a care watchdog, including safeguarding residents from abuse.
The Care Quality Commission (CQC) visited The Old Deanery in Braintree three times in July.
Its report, published last week, said enforcement action was taken on care and welfare of residents and assessing and monitoring service quality.
Action is needed to meet standards for respecting and involving residents, consent to care and treatment, safeguarding residents from abuse, staffing, supporting workers and records.
Anglia Retirement Homes, which runs The Old Deanery, has put a raft of measures in place to tackle the issues raised.
It was the second inspection held this year.
