Braintree: Reports highlight failings in run up to double murder

Two reports have been published on how agencies and authorities could have acted differently in the run up to two murders.
The reports look into the cases of two-year-old Shania, who was shot dead along with her mother Christine Chambers in Braintree in June 2011 by Christine's ex-partner David Oakes.
The Essex Safeguarding Children Board’s review looked into whether Shania could have been better protected
The report concluded in saying that “the specific incident was not predictable but there were a number of opportunities for intervention which if handled differently could have resulted in Olivia being considered more at risk”.
Another report, a Domestic Homicide Overview Report, reflects on the help given to Christine and concludes that she was failed by the legal system.
